The Cortisol & Cortisone reagents set from diagnotix • •• can determine the concentrations of both cortisol and cortisone in urine or saliva.

First an internal standard with D4 labeled cortisol and D8 labeled cortisone is added. All interfering substances are then removed using SPE columns, after which the eluent is evaporated to dryness. The residue is dissolved and can now be analyzed using the LC-MS.

The accuracy of the test is guaranteed with a 6-point calibration based on spiked humane 0 urine. This way the samples’ matrix can be simulated as much as possible. To continuously guarantee the quality of the test, it is possible to provide a 3-level control with this same matrix.

Clinical relevance

The ratio between cortisol and cortisone is of clinical importance with the diagnosis of Cushing syndrome. This even indicates apparent mineralocorticoid excess, a disease where the 11β-hydroxysteroid dehydrogenase-activity is weakened significantly or is even absent.
